What's the difference between the injury designations?

There's Questionable, Probable, doubtful, and Out. What do these signify? Do the names mean what they say, just tell you how the team is evaluating them?

The official meaning is this: Out = 0% chance of playing, Doubtful = 25% chance, Questionable = 50% chance, Probable = 75% chance.

So I guess players on these list do not mean anything when it comes to the final selection for Sundays?

Edit: I don't mean it that way, do each guys on the different lists have the same chance of playing on Sundays?
