5 straight losses after starting 6-2. Cam 'Fig' Newton threw a pick down 6 with a minute left against the Brownies.

Off the top of my head, the two worst flameouts I remember were when the '93 Dolphins started 9-2 and lost their last 5 to miss the playoffs, and the '03 Vikings started 6-0 and finished 3-7 to miss the playoffs.

Don't forget the 2013 Lions. They were 7-5 at the end of November and in 1st place in the NFC North. They lost their last 4 games to finish 7-9.
