Is the kick return defense a schematic issue?

MM said it was the players fault and not on coaching. Maybe this is true for the fumbles but surely the horrid returns could be defended better? Is Harbaugh responsible?

Harbaugh and all the coaches are to stay clear of the playing field when the ball is in play.

I would have to see the all-22, but it looked like the coverage team didn't maintain lane discipline on the return for TD. Schematic, but on the players for not sticking to the scheme.
