Multiple times. The Vikings did it last in 2016. Started 5-0, finished 8-8. However, no team since the merger has started better than 5-0, been the last undefeated team, and missed the playoffs. A few teams pre-merger managed 6-0 in those circumstances, but the only team to start better than 6-0, be the last undefeated team, and miss the playoffs were the 1967 Baltimore Colts, who started 11-0-2 and lost the last game of the season to the Rams, who were in the same division and had a 10-1-2 record going into the game. They had tied earlier in the year, and only the division winners qualified for the playoffs at the time, so the Colts sat at home during the playoffs with an 11-1-2 record, which would have been good enough to win any other division.
