Ringworm Sherm was the first official one from one of the earliest Real Rob Reports, if memory serves.

Personally, I love the nickname that a lot of 49ers fans use for him. Dick Sherman. I say it with love, but they say it enough anger and hatred to make Emperor Palpatine blush.

If I was a Niner fan I would look for a different nickname for him. If you go by their nickname then the last time Seattle played the Niners the game ended by Dick giving them the tip. But to each is own...

Wilson did call Sherman "Optimus" as the Super Bowl was winding down (as shown on the SoundFX). So that caught on a little within the team. But I see no need for him to have a nickname unless one actually comes about organically.
